Dear valued EyeQ customer,

As mentioned in our previous message to you on 30 October 2024, Luxottica Retail Australia (Luxottica) acquired the EyeQ Optometrists network (EyeQ Network) on 31 January 2024. We want to reassure you that everything at your EyeQ practice will stay the same. This means that all records held by EyeQ, including your health information, will be transferred to Luxottica’s systems unless you choose to opt out.

What are your options?

• If you’re happy with the transfer of your health records to Luxottica, no action is needed
• If you’d prefer not to transfer your health records to Luxottica, please email us at privacy@eyeq.com.au by 25 January 2025 with the name(s) and mobile number(s) of anyone objecting to the transfer. Please also indicate whether you would like your records sent to you or your preferred healthcare provider. If you don’t nominate where you would like your records sent, EyeQ Network will securely destroy your health record, to the extent permitted by law.
  • ACT customers: If you don’t nominate where to send your records, they will be transferred to Luxottica.
  • NSW customers: If you don’t nominate where to send your records, they will be archived by Luxottica on behalf of EyeQ, in accordance with mandatory health record retention requirements. To access these records, please contact us.

Luxottica will continue to honour your current EyeQ marketing subscription. You can update your marketing preferences at any time by emailing privacy@eyeq.com.au.
